2. If you pay by mailing, please mail the original bank-in slip or cheque to
HKTraveler before 5th December, 2006.

with the original bank-in slip or cheque and original
declaration before 5th December, 2006
Application fee:
★ HK$400 per head for individual
participant and 2-member team
< HK$100 donation to Children’s
Heart Foundation is included>
★ HK$1,600 per 4-member
team (for enterprise or organization only)
and the minimum HK$1,000
fund raised for Children’s Heart Foundation for charity purpose, which has to
be paid during application.

Letter of confirmation will send to participants by email if application succeed. |

Those participants who do
not pay by credit card or apply by mailing must
send the original bank-in slip or cheque and the
original declaration to HKTraveler on/before 5th
December 2006 (according to mail stamp), otherwise,
your application will not be processed. |

The preferential application
fee ($350) is only applicable to those participants
who settle the fee on or before 31st October 2006.
If participants pay on any day between 1st and
5th December, the application fee will be $400,
regardless whether the applicant applies on-line
or by mail on or before 31st October., 2006 |

If there is insufficient information or absence of bank-in slip and original declaration in mailing application, such application will be handled as reservation. |

Under no circumstance will the application fee and charity donation be returned. |

Fund raising
Further donation to Children's Heart Foundation depends
very much on your initiative to help those children with
congenital heart diseases. All donations will have donation
receipts for tax reduction, and prizes are reserved by
the organizer to encourage donation. Please download
the “Donation Form” below, thanks you!
Please note:
Confirmation email for participations will be delivered after the organizer receives
all necessary document, including application form, application fee and declaration.
If participant cannot receive the confirmation email latest by 11th December
